All non-golfers must register for the dinner below.
The cost is $75 per person and includes dinner, entertainment, and open bar. $30 of which will go towards supporting the mission and is tax deductible. In addition, all non-golfers who are registered for the dinner will be able to participate in an optional Paint n' Sip activity prior to dinner beginning at 3:00pm. Please indicate if you will be participating on the form below.

When you click "Proceed to Online Payment" it will take you to a separate, secure Stony Brook Foundation website to make your payment. Please calculate the cost ($75 per person) and enter it in the "Other Amount" box. Select "One-time Gift."
Next type 'Dental Mission to Madagascar' in the search bar for "Who Would You Like To Support."
In the "Tribute Information", enter your name in the "Name of person being honored" box. Select tribute type "In honor of," and enter "Golf Tournament" in the description.
